ADF Helicopter Crash Sparks Inquiry: Safety Concerns, Delays, and Grieving Families
October 18, 2024Fifteen months after the crash, Scells has yet to receive necessary statements from the ADF, complicating the investigation.
During the inquiry, Major Ian Wilson, a former MRH-90 test pilot, expressed deep regret to the families of the deceased and emphasized that blame should not rest solely with the pilots.
On July 28, 2023, the MRH-90 Taipan helicopter, call sign Bushman 83, crashed into the ocean south of Hamilton Island during a training mission, resulting in the deaths of four Australian Defence Force officers.
An independent inquiry, led by former judge Margaret McMurdo, is currently investigating the incident and the operational environment at the time of the crash.
Another aircrew member, known as D7, shared a personal near-crash experience to highlight the dangers associated with the MRH-90.
The ADF is conducting its own internal investigations alongside military police and the Defence Flight Safety Bureau (DFSB) to uncover more details about the crash.
The deceased soldiers were Captain Danniel Lyon, Lieutenant Maxwell Nugent, Warrant Officer Class Two Joseph Laycock, and Corporal Alexander Naggs.
Prior to the crash, Australian Defence Force aircrew expressed 'little confidence' in the MRH-90 Taipan helicopters, raising concerns about their safety.
In December 2023, the ADF retired the MRH-90 fleet as investigations into the crash and its causes continued.
Wilson previously warned officials about potential industrial manslaughter charges if safety concerns regarding the MRH-90 were not addressed.
The inquiry is facing delays in obtaining witness statements, which are crucial for the ongoing coronial investigation.
Detective Sergeant Luke Scells, leading the coronial investigation, reported difficulties in gathering statements from key witnesses.
