The inquest aims to explore whether early intervention could have prevented Cauchi's mental health from deteriorating prior to the attack.

Following the attack, Cauchi's parents recognized him from CCTV footage and assisted police in the investigation.

The victims included Ashlee Good, who fought off Cauchi while he attacked her nine-month-old daughter, and security guard Faraz Tahir, who was fatally injured while trying to help others.

During a preliminary directions hearing held on November 12, 2024, counsel assisting the inquest emphasized the rarity of mass casualty events in Australia and the need to identify systemic deficiencies to improve emergency responses.

Concerns were raised about the effectiveness of the security measures at Westfield Bondi Junction, as no alarm was sounded until approximately one minute after police shot Cauchi.

Prior to the attack, Cauchi had contacted police about concerns regarding his mental health and his collection of knives, which raised alarms among his family.

Joel Cauchi, who had a long history of schizophrenia, ceased taking his antipsychotic medication nearly five years before the deadly attack on April 13, 2024, at Westfield Bondi Junction.

The inquest will investigate Cauchi's interactions with police in Queensland and New South Wales, as well as the response time of the alarm system during the attack.

Cauchi's attack resulted in the deaths of six individuals, including four women and a security guard, while injuring ten others, with many of the victims being female.

The full inquest is scheduled to take place from April 28 to May 30, 2025, focusing on Cauchi's mental health treatment, police interactions, and security responses.

NSW State Coroner Teresa O'Sullivan extended condolences to the families of the victims, acknowledging the ongoing pain and trauma from the incident.

On the day of the attack, Cauchi accessed a storage locker and used public transport to reach Westfield Bondi Junction, where he began his assault shortly after entering.
